City of Yukon forms

Concrete driveway Yukon: the City's curb-cut permit, the pre-pour inspection, and the street side

A Yukon driveway starts with the City of Yukon curb-cut and driveway application, the inspection that form requires before a pour, and the separate right-of-way application for work in the public street. Call or text 405-913-6183 with the street address and photos of the drive and the curb.

Curb cut / driveway application

What the curb-cut form asks you to mark

The City form is titled “CURB CUT / DRIVEWAY PERMIT APPLICATION.” Under requested permits it has two boxes: Curb Cut, and Driveway. The top of the form asks for the location of work, the applicant, the owner, and the subdivision, lot, and block.

Under “TYPE OF PROPERTY AND FEES,” the form prints residential $75.00, commercial $150.00, and industrial $150.00. Those are the City's permit fees, separate from the cost of the concrete. The material boxes are Concrete and Asphalt. Under “CHECK ALL THAT APPLY,” the boxes are New, Alteration, Repair, and Addition. The form also has a blank for the estimated cost of the improvement.

The contractor block asks for a name, phone, and address, a “Contractor Registration #,” an expiration date, a project supervisor, and an email.

A site plan is required to accompany application showing existing buildings, fences and other structures along with the drive location and the location of other drives, streets, and alleys.

The form says the application must be submitted to the Development Services Department and approved prior to work being started. It also says a City of Yukon inspector must inspect all work. The footer prints “P. O. Box 850500 Yukon, OK 73085,” phone 405.354.6676, and www.cityofyukonok.gov.

Driveways and curb cuts located on state highways require that the applicant submit a completed State of Oklahoma, Department of Transportation driveway permit application with this document.

Notes for a lot inside Oklahoma City are on the concrete contractor in Oklahoma City page. Yukon notes that are not about these City forms stay on concrete work in Yukon. Driveway work apart from the forms is on concrete driveways.

Before concrete is placed

Call before the pour

An inspection request shall be made at least 24-hours prior to pouring concrete, after the proposed work has been formed and made ready to pour.

City of Yukon Curb Cut / Driveway Permit Application, Inspections section, read October 23, 2026.

For a curb cut or driveway inspection, the form says to call (405) 354-6676 and state the job address, the type of inspection you are requesting, and when you will be ready for inspection. It also says inspections are called in after excavations are made and form work is in place, but prior to pouring concrete.

The same section says it is the contractor’s or owner’s responsibility to see that the work is done in a professional manner, with all curb cuts saw cut. Barricades or other warning devices go where necessary, and the site is left clean when the work is finished. The form says: “Any sidewalk or other public improvements must be repaired or replaced if damaged during construction.”

August 2023 right-of-way form

Street work sits on a second form

Work in the public right-of-way is a different application: “Application for Permit to Construct within the Road Right of Way.” That form is marked “Updated August 2023.” The curb-cut form does not say where a private drive ends and the public right-of-way begins. Call (405) 354-6676 with the job address and ask which form that address needs.

All work within public rights-of-way shall be performed by a City of Yukon registered contractor with current insurance and comply with the City of Yukon Standards for Public Works Construction.

City of Yukon right-of-way application, Updated August 2023, read October 23, 2026.

That requirement applies to whoever does the street-side work.

The form asks for a general liability certificate with the City of Yukon as certificate holder, minimum $500,000 per occurrence, and a Workman's Comp certificate with the City of Yukon as certificate holder. The permittee shall provide a 2-year bond, in a form approved by the City and accepted by City Council: $1,500.00 for a cut in asphalt pavement, $2,500.00 for a cut in concrete, and $1,500.00 for a tunnel. The form cites City of Yukon Code of Ordinance Section 94-75. That section, as reprinted on the form, includes paying city charges, maintaining repair and backfill for two years, and correcting settling of cuts. It also says the bond shall not apply to franchise holders or utility companies paying a gross receipts fee to the city, but shall apply to their subcontractors.

Before digging, the form says to call Okie811 at 1-800-522-6543. Barricades and signage shall conform with the Manual of Uniform Traffic Control Devices, and all road or lane closures require 48-hour advance notice. All work shall be inspected by the City of Yukon Development Services Department. For those inspections the form says to call 405-354-6676.

The City’s Forms & Applications page links an earlier right-of-way PDF (document 131). The street-side figures here come from the August 2023 version (document 3409), which lists the same insurance minimum and bond amounts.

Before anyone files

Questions for whoever does the work

Ask these before the forms go to Development Services.

Registration number

The curb-cut form has a “Contractor Registration #” line and an expiration date. The right-of-way form has a City of Yukon contractor certificate registration number and an “INSURANCE ON FILE” box. Ask for the number and the expiration the form is asking for.

Who files which form

Ask who will file the curb-cut application, and, if the work is in the public right-of-way, who will file that second application. The curb-cut form says the applicant certifies they are the owner of record or the owner’s authorized agent. Filing is that person’s step.

Inspection timing

Ask when the inspection will be requested. The curb-cut form’s timing is at least 24-hours prior to pouring, after the work is formed and ready to pour, by calling (405) 354-6676 with the job address, the inspection type, and the ready time. Ask the same question for a right-of-way inspection, which uses that phone number on the August 2023 form.

What helps the first call

What to have ready when you call

Call or text 405-913-6183. Send the street address and photos of the drive from the street, the garage edge, and the curb. Say whether the boxes you expect to mark are curb cut, driveway, or both, and whether the drive is on a state highway. A written number comes from the lot — not a website square-foot rate.
